Up to now, when survey in surveyor pattern expired, xs_recv()
returned EAGAIN. That made it impossible to distinguish
epired survey from expired receive timeout (XS_RCVTIMEO).
This patch make errors different in both cases.

libxs (as opposed to libzmq) requires at least one I/O thread per
context. Thus, "no I/O thread available" error doesn't make sense
anyome.
This patch removes the code and the documentation.
It keeps the definition of EMTHREAD itself for backward compatibility.

This patch changes the Win32 version to call RaiseException instead of abort
(which eventually calls TerminateProcess). This allows crash dumps to be sent
correctly instead of the process disappearing.

This patch addresses serveral issues:
1. It gathers message related functionality scattered over whole
codebase into a single class.
2. It makes zmq_msg_t an opaque datatype. Internals of the class
don't pollute zmq.h header file.
3. zmq_msg_t size decreases from 48 to 32 bytes. That saves ~33%
of memory in scenarios with large amount of small messages.

C and C++ headers moved from bindings/ to include/, bindings/ removed
--with-c and --with-cpp options to configure removed, C and C++ now built
and installed by default
